• SAMHA Coaching Application Form.  If you would like to apply for a coaching  position for the current year of minor hockey, download and complete this form and submit it by mail to our mailing address above, or directly to our Coaching Coordinator.
• SAMHA Fund Raising Policy & Application Form.  Complete this form and submit it to the President for approval.  This application must be completed by each team and approved by the President for every fund raising program, and before the fund raiser is to take place.  Funds raised through SAMHA fund raising approved projects are to be used for approved activities or purchases only, and are subject to the rules set out in our Association policies.  Funds are not to be used for purchasing gifts for team officials, or for team parties or non-hockey-related events.
• Post-Season Parent Observation Form: Coaching (formerly Coach Evaluation form).  This form should be completed at the end of the current minor hockey season by parents of SAMHA players to provide feedback on program delivery by their child's coaches.  It should be used to provide constructive criticism and/or praise for our coaches, and to look at suggestions for improvements to our coaching program.  It will also be used to help evaluate potential coaching candidates for following seasons. Please download this PDF form and mail it to the SAMHA office, Attention: Coaching Coordinator, or give it directly to your Division Director.  Please complete and return one form for each of your child's coaches.  All personal information is held in confidence.
• SAMHA Team Roster Form.  Microsoft Excel or PDF versions.
• SAMHA Code of Conduct:  Team Officials Contract.   Every season, all team officials (coaches, assistant coaches, trainers, team managers, team equipment managers) are required to read, sign and date, and abide by the responsibilities and guidelines as indicated on this contract.  If you are a team official, please download this form here and present your signed contract to the Coaching Coordinator at the beginning of the season.
• SAMHA Tournament Application Forms and Instructions
• SAMHA Notice Of Motion form.  This MS Word document form should be used to submit a Notice of Motion by the annual submission deadline to adopt, amend, revise or repeal By-Laws or Regulations of this Association.
• Okanagan Mainline Amateur Hockey Association (OMAHA) Scholarship Application.  The deadline for submission of your application is April 30th of the current year.
